Who is MemoryWise? 

Tara Chapin is a licensed social worker with over 17 years of experience working in healthcare. She has worked in hospitals, nursing facilities, and doctor’s offices. Her experience is primarily with seniors, but she also has a passion for helping individuals managing a disability and families who are grieving. Tara is a Certified Dementia Practitioner and a mobile Notary Public in the state of Ohio.
